Good Faith Estimate for Health Care Items and Services:
Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
Rose LaPiere, LPC, is an out-of-network provider and does not accept insurance, so payment is due at the time the service is rendered. Rose will generate a statement that you can submit directly to your insurance carrier. Please check with your insurance to see if you are covered and what the reimbursement would be, if any. If you have health insurance, and the services you are seeking are covered by your health care plan, you may be able to get the items or services described in this notice from providers who are in-network with your health plan.

CANCELLATION POLICY for all Individual and Family Sessions
We have implemented a cancellation policy to benefit all of our clients. We truly value your time and ours and ask for a 24 hour notice for schedule changes and cancellations so that we can provide that time slot to another person in need. Please give at least 24 hours notice of your scheduled appointment or you will be charged the full fee of the session.
